What is personalised care support at home?

Personalised care and support planning involves:

Personalised care and support is a process that helps the individual receive integrated health and social care services. It’s based on ‘What Matters to the Individual?’ by recognising and empowering their strengths and needs.

Personalised care and support planning involves:
  • Collaboration
    • - Especially if the individual has long-term conditions, carers and family must work together to understand what’s important to the individual.
  • Goal Setting
    • - Individuals, NOK and care professionals must agree on goals and identify support needs.
  • Action Plans
    • - individuals and care professionals must develop and implement action plans to achieve the goals.
  • Progress Monitoring
    • - individual and care professionals must monitor progress.

    What is live-in carer?

    A live-in carer is

where a professional carer lives with you or your loved one, allowing them to stay for as long as needed. According to a care market survey, live-in carers are one of the largest and growing areas in the care market as many people, families, and care professionals recognise the Home First approach is the safest option because of the proper support, comfort and familiar environment that carers can provide at home.

"A professional live-in carer ensures that you or your loved one receives exceptional care and support at home while maintaining dignity and retaining independence."
